Enhanced Care Management, is a new Medi-Cal benefit to offer medical and non-medical services. Our ECM Care Managers will meet with you to discuss the type of support you need.
How to Qualify
- Currently enrolled in or eligible for Medi-Cal. If you don’t have insurance, we can help you enroll.
- Currently or previously in foster care (up to 26 yrs. old)
- Reside in LA County
- Under 21 years old for non-foster youth with complex medical, mental or social needs or facing homelessness
Obtaining Housing, Food and Free Transportation to Appointments
Finding Doctors, Receiving Medication and Mental Health Support
Substance Use Recovery
Pregnant or Postpartum Services
Help In Finding Education and Employment Opportunities
