CFGC’S COMMUNITY RESOURCE SERVICES WORK WITH MEDICAL DOCTORS TO ACHIEVE EXCELLENT PATIENT OUTCOMES
Did you know that medical doctors can work with CFGC’s Community Resource
Service (CRS) to help them ensure that their patients stay as healthy as possible?
CRS medical educators accompany patients to their doctor appointments and
work side-by-side with doctors to make sure patients are consistently managing
their serious health issues. Families also benefit from having a CRS case manager
to help them navigate the often confusing healthcare system.
Our case management team helps families gain access to quality healthcare
from MDs by removing barriers that keep them from seeking treatment for
health problems. The service also provides medical doctors’ patients with links
to community support resources that work closely with MDs to provide services
that families need the most. Ultimately, CRS reduces tasks that impact MDs’
ability to treat more patients.
